​Croasdaile Village, the Triangle’s premier life-plan community, offers residents a continuum of care that includes independent and assisted living residences, an on-site health clinic, and a skilled nursing center if the need ever arises, all on campus. There is never a need to seek new housing later on if you ever need more assistance.

No matter which level of care our residents need, our staff focuses on providing the absolute greatest level of independence in a secure environment where assistance is always available.

We also accept Medicaid and Medicare to allow you worry-free care. Because our Heathcare Center offers closed beds, residents are guaranteed assistance when they need it at the place they call home.

Home Care

Our Home Care service provides warm personal care and assistance allowing you to remain at home and independent. We can provide 24-hour home health services, skilled nursing visits, medication management, meal preparing assistance, and more. We strive to tailor our services to you so that you can lead the most complete and fulfilling lifestyle possible.

Clinic

Our on-site Clinic can become your primary care physician's office if you choose. The Clinic Medical Director is a Geriatric Physician on Staff at Duke who comes to the Clinic one day each week for annual physicals and to see clinic patients. The Clinic is manned five days a week with nurse practitioners who can diagnose and prescribe, and an RN who can treat ailments and illnesses. Treatments based on your doctor’s orders can also be performed or monitored.

Health Care Navigator

All Croasdaile Village residents have access to a Health Care Navigator. The Navigator provides guidance to help you reach your health and wellness goals. For example, the Navigator can coordinate appointments with our registered dietitian for a nutritional consultation or a member of our fitness team to find activities that are right for you. The goal of the Navigator is to assist you in maintaining overall wellness, and help you avoid admission to higher levels of health services.

Should your health care needs change, the Navigator also acts as a liaison for you and your family, helping to schedule health and medical related appointments and services. If you have to go to the hospital for a procedure or due to illness, the Navigator can assist with coordinating transfers and paperwork.
