Croasdaile Village Navigation & Home Care ‘Fit Hand in Glove’

In just over a year, the combined Navigation & Home Care Department has working together effectively under the leadership of Kim Broucksou to meet the needs of Croasdaile Village residents for short or long-term assistance with their care.

On the Navigation side, two social workers help independent living residents and their family members understand and navigate through the medical and support services that are needed here to to transition successfully from one level of care to another. “It is never too soon to contact the Navigation team to start to understand and plan for the road ahead,” says Kim, adding “The more they know your values and situation, the better they can plan for a resident’s future and react to the unexpected.”

The Home Care team of professionals is dedicated to providing quality service across all parts of our continuing care retirement community. Programs have been tailored to provide services and caregivers to residents in order to ensure their most fulfilling lifestyles possible and give their families true peace of mind. “Whether you need 24-hour around the clock care, skilled nursing visits, medication management, meal preparation or simply someone to accompany you on a walk around the lake, we can provide the care residents need,” says Kim.

The department’s 80 Certified Nursing Assistants help individuals with activities of daily living such as: bathing, dressing, transferring from one place to another, feeding, shopping, light housekeeping, and a variety of tasks to help residents live as independently as possible.

RNs perform all skilled nursing tasks in your home including patient assessments, dressing changes, medication management, wellness monitoring, and following the orders of a resident’s doctor. The case RN also reviews each residents needs at least every 60 to 90 days and communicates to caregivers how residents want and need to be cared for.

Kim points out, “Most residents probably don’t know that Croasdaile Village Home Care is a state-licensed home care agency. This is really important when it comes to residents’ long-term care insurance paying for services. Almost all long term care insurance companies ask that home care services be delivered by a licensed agency. We get the documentation and the invoices to the insurance company so residents get reimbursed for the services they pay us for.”

One particularly challenging part of running the Home Care department is that almost all of the CNAs work is on a per diem, or as-needed basis. Says Kim, “Most of the aides that work with us have full-time or other part-time jobs somewhere else. Some aides work here to supplement their retirement income. That means that the aides tell us what availability they have and we then try to match them up with service requests from the residents.” In addition, the employment door is ever-revolving as Kim is in a continuous hiring mode to replenish the aides that may find other jobs or just can’t work the hours we need. She will hire, on average, three aides a month and also see about three fall off the roster.

Kim Broucksou obtained her Nursing Home Administrator’s license in 2016 and came to Croasdaile Village in 2018. A 20 plus-year veteran in long-term care work, Kim has worked for hospice, free-standing nursing homes, non-profit volunteer agencies, and for over 12 years at another nearby CCRC facility.

Maryellen Stone joined CV’s Home Care Department in June of 2013 and was promoted to Executive Assistant in 2018. Home Care’s RN Case Manager Felicia Izzard’s nursing experience spans 25 years before coming to Croasdaile Village in 2017. Home Care Schedulers helping get caregivers and residents connected for appointments include Jennifer Myers and Janice Cobb.

Lori Leathers came to CV nearly four years ago with more than 20 years of social work experience. Responsibility-wise, Lori works with residents on Boardman, Susanna, Park Homes K,L,M,N, and Homestead Ground, 1st and 2nd floor. Caroline Ryskiewhich became a Navigator a year ago, previously bringing much joy to our residents since her arrival here in Life Enrichment at Croasdaile in 2020. Caroline works with residents from Aldersgate, Barratts’ Chapel, Coke, Davisson, Garrettson, Garber, Assisted Living and Homestead 3rd floor.
